Dear parent,
Please read Terms and Conditions if you would like your child or children’s to be enrolled in Saba Center’s Soccer Program.
I, the undersigned parent/legal guardian, hereby give consent for my child/ward to participate in Saba Center’s Kids Soccer Training Program. I acknowledge the physical nature of soccer and its inherent risks. By signing this agreement, I agree to the following terms and conditions:
Digital Signature Agreement: I understand and agree filing out forms in this page or on payment links, it is equally legal binding and forceable as signing manually.
Assumption of Risks: I recognize that soccer involves risks such as injuries from falls, collisions, and equipment-related incidents. I accept these risks on behalf of my child/ward and acknowledge their participation is at their own risk.
Medical Conditions: I confirm that my child/ward is healthy and fit for soccer activities. I will inform Saba Center of any existing medical conditions, allergies, or other relevant health information that may impact their participation.
Supervision: I understand Saba Center will endeavor to ensure participant safety. However, I acknowledge they are not liable for accidents, injuries, or lost property during training sessions.
Release of Liability: I release and hold harmless the Saba Center Kids Soccer Training Program, its organizers, coaches, staff, and affiliates from all claims or liabilities related to my child/ward’s participation.
Medical Treatment Authorization: In emergencies, I permit Saba Center to seek necessary medical treatment for my child/ward. If I cannot be reached, I authorize them to act in my child/ward’s best interest.
Photography and Media Release: I allow Saba Center to use photos or videos of my child/ward for promotional or educational purposes in print and online media.
By submitting my information and filling out the payment link on the next page I confirm my understanding and agreement to these terms and conditions as well as Terms and Conditions set on the Terms and Conditions page, waiving any right to claim or take legal action against Saba Center or its affiliates.
